Apply for driver's license validity extension for professional vehicle drivers (reaching 60 years of age) of sedan sized cars
- R.O.C.-I.D. card (original).
- The original driver's license.
- Physical examination chart (including X-rays and E.C.G.), conducted at public hospitals or NHI bureau appointed hospitals; all such applicants must pass relevant physical examination.
- Three identical recent 1-inch bust front view photos, bareheaded, on glossy paper (including physical examination).
Footnote：When reaching 60 years of age, those professional vehicle drivers who wish to continue, will only be issued temporary driver's license for small sedan sized cars, validity 1 year at a time; maximum age limit permitted is 65 years old.
